veliler formu imzalaması gerekmektedir .

2 Yalnız ya da tek ebeveyn veyahut yasal vasi ile seyahat eden 18 yaşın altındaki çocuklar için;
-Seyahat etmeyen ebeveyn/yasal vasinin noter onaylı yazılı izni (muvafakatname) ya da çocuğun velayetinin seyahat
eden ebeveyn/yasal vasiye verildiğini gösteren noter onaylı belge (Son 3 ay içinde alınmış olup hem original hem
fotokopi olarak sunulmalıdır.) (Geçerli vizesi olsa bile başvuru yapmayan her ebeveynden muvafakatname)
Minor under the age of 18 years and travelling alone or with one parent or legal guardian only:
-written consent (approved by a public notary) by the non -travelling parent(s)/legal guardian(s) or proof (approved by a public
notary) of sole custody of the travelling parent/legal guardian. (Must be obtained within the last 6 months and submitted both
original and photocopy) (Consent letter from each parent who does not apply even if they have a valid visa)

Travel arrangments : flight reservation, other proof of intended means of transport, or proof of travel itinerary
5 Konaklamaya dair kanıt: otel rezervasyonu ispatı ya da konaklamaya ilişkin diğer kanıtlar
Proof of accommodation: evidence of hotel booking or other proof of accommodation.
6 a) Başvuru sahibi Çalışan ise;
-İşveren tarafından yazılmış; hangi konsolosluğa hitaben yazıldığı bellirtilmiş; çalışanın adını, pasaport numarasını, işe
başlama tarihini ve şirketteki görev bilgilerini içeren; izin tarihlerini, ücretli ya da ücretsiz izin olup olmadığını belirt en;
işverenin iletişim bilgilerinin, yazıyı imzalayan kişinin isim ve mevkiinin yazıldığı izin belgesi
-SGK (sosyal güvenlik) Sigortalı İşe Giriş Bildirgesi ve SGK tescil ve hizmet dökümü QR kod lu okunabilir olmalı
b) Çiftçi ise;
-Ziraat odası tarafından verilmiş çiftçi belgesi
c) Şirket sahibi ise;
-Şirketin ticaret odasındaki kaydı ve ticaret sicil gazetesinin kopyası
-Vergi levhası
-Faaliyet belgesi
d) Öğrenci ise;
-Yüksek öğrenim: YÖK tarafından verilen okunabilir QR kodu bulunan öğrenci belgesi
-Diğer öğrenciler: öğrenci belgesi
a) Employees:
-letter from employer and/or approval for leave containing the following information: indication of the consulate that the doc ument
is addressed to; the employe e’s name and passport number; date of start of employment and function; length of the leave and
whether it is paid or unpaid leave, employer’s contact details; name and position of the person signing the letter.
-SGK (social security) statement of employm ent (Sigortalı İşe Giriş Bildirgesi) and SGK registration and service document (SGK
tescil ve hizmet dökümü) with a readable QR code.
b) Farmer:
-farm certificate issued by a chamber of agriculture.
c) Company owners:
-the company registration in the chamber of commerce and copy of the bulletin of the trade register. -statement
of taxes payment.
-company activity certificate (Faaliyet Belgesi).
d) Students:
-Higher education: student certificate issued by the Council of Higher Education in Turkey (YÖK) with a readable QR -code. -Other
students: student certificate
7 Geçim kaynağını gösterir belgeler a) Son 3 aydaki hareketleri gösteren ve düzenli gelir kaynağını kanıtlayan banka hesap
dökümü ve b) son üç ayın maaş bordoları (veya benzeri) gib düzenli geliri gösterir belge, c) uygun olan durumlarda;
emekliliği gösterir belge
Proof of means of subsistence a) bank account statement showing movements over the last 3 months, proving the source of
regular income, an d b) proof of regular income, such as salary slips of the last three months (or similar), c) pensioner booklet if
relevant
8 35×45 mm ölçülerinde son 6 ay içinde çekilmiş, arka fonu beyaz olan 1 adet fotoğraf (şahsen başvuru yapanlar için
fotoğrafa gerek yoktur.) (12 yaş altı her başvuru sahibi için fotoğraf (2 tane) zorunludur.)
1 photograph taken in the last six months, 35×45 mm in size with a white background (photo is not needed for applicants who
apply in person) (All applicants under the age of 12 should submit two photographs)
9 Pasaport fotokopileri (bilgilerin ve vizelerin olduğu sayfalar)
Pasaportunuz belirlenen dönüş tarihinizden sonra en az 3 ay geçerli olup son 10 yıl içinde alınmış olmalı ve en az 2 boş
sayfa içermelidir.

Passport should be valid for at least 3 months beyond the intended date of departure from the territory of the Member States and
must be issued within the last 10 years and contain at least 2 blank pages
10 En az 30,000 Euro teminatlı, tüm Schengen ülkelerinde geçerli ve bütün seyahat süresini kapsayan seyahat sağlık
sigortası
Travel health insurance with a minimum coverage of 30.000 Euro, valid for all Schengen Member States and cover the entire
period of the person ’s intended stay
11 Irak vatandaşları kimliklerinin orjinalini göstermeli+ bir kopyasını tercümesiyle birlikte sunmalıdır.
Iraqi citizens must show their National ID-card in original + submit a copy along with its translation
12 Yabancı uyruklu başvuranlar;
– İsveç vizesine başvuru yapabilmeleri için son bir yıldır Türkiye ’de ikamet iznine sahip olmaları gerekmektedir. (Bu gereklilik Iraklı,
Afganistanlı, Libyalı ve Suriyeli başvuranlar için geçerli değildir.)
Pasaportun seyahat dönüş tarihinden itibaren 3 ay daha geçerli olması gerekmektedir.
3. ulke vatandası olan başvuru sahipleri, Turkiyede ikamet ettiğini gösteren bir belge sunmalıdır. Bu belgenin geçerlilik sür esi , Üye
devletlerin topraklarından planlanan ayrılış tarihinden itibaren en az 3 ay daha geçe rli olmalıdır.
Non-Turkish nationals:
– Foreign nationals must have had a residence permit in Turkey for the past year to apply for a Swedish visa. (requirement does
not apply to Iraqi, Afghani, Libyan and Syrian applicants)
Proof of passport valid thr ee months beyond the intended date of departure from the territory of the Member States.
A third national must also present proof of residence in Türkiye, valid three months beyond the intended date of departure fr om
the territory of the Member States.
